Scholars Athletics Clearance

 

Note:

 

As a reminder scholar athletes need to submit an initial sports/athletic packet for the first sport they are participating in. After the initial sport, they will need to submit a supplemental packet for each additional sport. The reason for this is to ensure scholar athlete insurance information is up to date. If you have any questions, please do not hesitate to contact the office.

The California Interscholastic Federation and the Monterey Peninsula Unified School District requires that a student participating in any extra-curricular sports activity must have a physical examination once a year. The High School Physical Examination Form MUST be completed by the examining physician. No substitute will be accepted.

 

The cost of the Athletic Insurance is the responsibility of parents/guardians. For those that do not have insurance, an insurance brochure form the Myers-Stevens Company is available at school for request.

Students will not allowed to participate in any sport without proof of insurance coverage.

 

 

If your son or daughter is new to the High School a transfer form and proof of residence will be required.
